Adult Family Care Program
The
PRIDE Adult Family Care program provides an alternative residential option
for individuals with disabilities who desire to live in the community
with a family. This option is for individuals who want a family placement
and would benefit from being with a trained careprovider. Many of these
individuals have serious, chronic medical conditions. They may need assistance
with personal care such as dressing, bathing, or walking. The placement
could be either short term or on a permanent basis. The AFC program will
improve the quality of life for the individual while becoming a productive
and valued member of their community.

Eligibility Criteria
An individual must be 16 years or older to participate in the program.
He/she must have Medicaid and have a medical condition that needs support. In general, the
participant must want to live with a family and need at least minimal assistance on a
daily basis with personal care.
